I am enrolling my child(ren) in Music Steps and do hereby release and discharge Music Steps and its employees, from any and all actions, claims, and demands for upon or by reason of damage, loss or personal injury which may be sustained by my child(ren) or myself during the course of or as a result of the above program.

Important Payment Information:
A deposit of $80 must accompany the registration form to guarantee your child’s space for next semester. Balance is due in a lump sum or in a maximum of 3 post-dated cheques (for 10, 15, or 30 week programs only) providing they are dated within a 3 month period with the first dated cheque beginning at the start of the semester and are submitted on or before the first day of class (no exceptions). An invoice indicating balance due and payments made will follow as your confirmation of receipt of this registration. THERE ARE NO MAKE UP CLASSES.

Cancellation Policy:
The deposit is non-refundable, unless otherwise
approved by the director. A charge of $30 will be levied for NSF cheques. Materials once opened are non-refundable.
We reserve the right to cancel classes due to insufficient enrolment. FULL refunds will be given.
